Care Community Hub Team

Ghana Health Services, Grammeen Foundation, Concern Worldwide

Although Ghana has experienced significant progress in health over the past several years, much work remains in improving the health condition of its vulnerable population, especially among the poor and those living in rural areas. Community Health Nurses (CHN) within the Ghana Health Service are key care providers in the healthcare infrastructure.

Research with the nurses indicated the need for a cultural change within the Ghana Health Service across multiple areas of concern. The result was an on-the-go app that has helped Ghana Health Service improve communication, performance and motivation of community health nurses. The app includes a learning center which offer courses that build clinical skills. Nurses can communicate with peers to share best practices, a point of care center helps nurses calculate dosage for medications and review symptoms.

While total financial value is challenging to quantify, the positive culture change and tools developed will have significant positive impact on the ultimate beneficiaries of the services: over 20,000 women and children who utilize Maternal, Newborn and Child Health services.
